San Diego Post-Acute Center’s facility is designed for individuals who have been recently released from the hospital. The experienced team provides personalized programs to help those who have been newly discharged, to individuals in a more medically fragile state. The 240-bed facility provides a wide variety of short-term and long-term services, including post-acute rehabilitation, sub-acute care, memory care, respite care, social services, as well as a hospice program.
